Working with the HFHT, dp.Ai coordinated the Transitional Funding projects for Family Health Teams for over a dozen locations.  Practices ranged in size from a sole practitioner’s office to large clinics with multiple physicians and allied health professionals.

The dp.Ai team was involved in all aspects of the process from the initial applications and feasibility studies, through the Ministry of Health and Long Term Care funding approval process and managing the construction and closeout of the projects.
